MANDATORY ARENA FORMS - The Zurich arena requires everyone entering the building to complete a screening form (this is separate from our OMHA screening at the door) as well as a liability waiver. These forms must be completed for every ice time. Forms can be found below - printing them off and filling out in advance will save time at the door.
